If you have obtained apex gear directly from an apex dungeon it can not be upgraded further 

If you have upgraded gear from level 80 then it can be upgraded further and will be stronger overall then gear that drops directly in the dungeon 

Bear in mind you need dropped gear for breakthrough when making upgraded gear so it will take a long time
